What To Bring

What to Bring

Please bring the following with you to your appointment:

All of your glasses including sunglasses

Contact lens boxes

Insurance cards, medical, secondary, & vision coverage

If you take several medications, please bring an updated list

A copy of your completed Patient Registration Form.
Payment as it is expected at the time of visit for eyewear purchases, contact lenses, and copays.

Please Note:

DILATING DROPS: If it is your first eye exam in our office, the doctor will probably put drops in your eyes to dilate your pupils. This allows the doctor to get a better view of the inside of the eye. The drops make you sensitive to light and last 2 -4 hours. If you are sensitive or will be coming from a distance, it is advisable to bring someone with you as a “driver”. Examinations with the doctors are typically by appointment. You do not need an appointment to pick up glasses or contact lenses, but it is a good idea to call before you come so we can be better prepared to serve you in an expeditious manner.

Routine eye appointments are the easiest and best way to keep your eyes healthy and to maintain clear vision. It is recommended that you have an eye examination every 12 to 24 months, based on your age and health. If it has been longer than two years since your last eye exam, or you are experiencing changes in your vision, please schedule an appointment now. You only have one set of eyes and we want to take care of them.
